An Aging Life Care Manager®, also known as a geriatric care manager, is a health and human services specialist who acts as a guide and advocate for families who are caring for older relatives or disabled adults. The Aging Life Care Professional is educated and experienced in any of several fields related to Aging Life Care/Care Management, including, but not limited to nursing, gerontology, social work or psychology, with a specialized focus on issues related to aging and eldercare.

Home Care Solutions Aging Life Care Managers assists clients in attaining their maximum functional potential. The individual’s independence is encouraged, while safety and security concerns are also addressed. Home Care Solutions Care Managers are able to address a broad range of issues related to the wellbeing of their client. They also have extensive knowledge about the costs, quality, and availability of resources within their communities.

Working with families, the expertise of Aging Life Care Managers provides answers at a time of uncertainty. Their guidance leads families to the actions and decisions that ensure quality care for loved ones, reducing worry, stress and time off of work for family caregivers.

Professional Aging Life Care Managers help counsel and support individuals and families who are overwhelmed by care decisions. In this unique model, the Care Manager develops a personalized plan, including the right mix of services, to ensure that a client’s needs will be addressed all while honoring their values and independence. The array of client services might include the following:

  • Consult, assess and make recommendations for care
  • Coordinate medical appointments
  • Manage crisis situations
  • Act as liaison to families at a distance who need a local professional they can trust
  • Evaluate residential options

DESCRIPTION OF SERVICES

Our expertise is helping families with challenging issues. As professional Aging Life Care Managers, we provide an objective assessment and access to a broad range of resources. Consultation with a Care Manager can provide the support for problem solving and decision making that families need when faced with eldercare dilemmas.

We offer a variety of individualized services to assist older adults, their families, and professionals with care planning decisions. We apply our experience and clinical social work skills to assist families in navigating the maze of medical, financial and long-term care options. Examples of our Aging Life Care Management Services include:

  • Provide proactive solutions that relieve the stress of caring for elderly or chronically ill family members
  • Explore options with families for medical and residential care including adult day programs, assisted living and nursing home choices
  • Provide recommendations for navigating Alzheimer’s and dementia care
  • Schedule doctor’s visits, coordinate medical care, attend medical appointments with clients and share the results with families
  • Manage crisis situations such as accompanying a client to the ER
  • Assist with long-term care insurance benefits process
  • Recommend safety equipment and specialized supplies for the home

Consultation: In-Office Family Meeting

Explore your caregiving concerns, map out possible options, and receive up-to-date information about eldercare resources available in the area including resource information.

Comprehensive Assessment: In-Home Visit

Through the assessment process, we obtain an in-depth review of the elder’s abilities and needs, including physical, functional, cognitive and home safety issues. As part of the assessment, we collect family input and concerns. The assessment results in a review of options and guidance for home care and long-term care planning. Typically, meetings last two to three hours. Separate family meetings can be arranged as needed. Optional written summaries are available at an hourly rate.

Care Plan Coordination

Your Care Manager can also coordinate the services needed. This may include researching specific resources for care, locating appropriate care options, coordinating services or making referrals.

Ongoing Care Management and Evaluation

Receive ongoing Care Management home visits for monitoring and crisis management, coordination of services as needs change, reports and updates to families.
